NOTICE OF PRIVACY POLICY FOR SPECIAL NEEDS ADVOCATE FOR PARENTS
This privacy policy notice is delivered to you from Special Needs Advocate for Parents ("SNAP"), in compliance with federal law including the Gramm-Leach-Bliley Act and its implementing regulations. We may make changes to the privacy policies set forth in this notice at any time and will provide you with any notice of such changes as may be required by law. We would be glad to discuss any questions you have regarding these privacy policies.
Under the Gramm-Leach-Bliley Act, "nonpublic personal information" is defined as personally identifiable financial information that we may obtain from you, lists of consumers derived from personally identifiable financial information, and any publicly available financial information that we obtain by using the information you provide. SNAP collects nonpublic personal information about you from information we receive from you on the questionnaires or other forms, such as name, address, telephone number and other contact information. This information may be collected from you in written form, by phone, on-line, or by mail, as well as from others.
SNAP may disclose all of the nonpublic personal information that we collect, as we described above, to the following types of third parties:
1. Financial service providers with whom SNAP has an existing relationship for the provision of services for families and individuals with special needs, such as insurance companies, brokerage firms, or financial service companies;
2. Nonaffiliated third party service providers who need to know that information in order to perform services or functions on our behalf, and who have agreed to hold your nonpublic personal information in confidence; and
3. Other parties as permitted by law or at your own request.
Right to Opt Out
If you prefer that we not disclose your nonpublic personal information to the above described financial service providers or nonaffiliated third parties, you may opt out of those disclosures (other than disclosures permitted by law or when you ask or give us permission to do so). If you wish to opt out of nonpublic personal information disclosures to such financial service providers or nonaffiliated third parties, you may note on any questionnaire: "No, do not disclose any of my personal information."

Security Procedures
SNAP restricts access to your nonpublic information to those employees and volunteers who need to know that information to provide SNAP's services to you. SNAP maintains physical, electronic and procedural safeguards in compliance with applicable laws and regulations to guard your nonpublic personal information.
