The Process:
- Initial discussion with SN
AP about the problem and whats been done so far.
- Parents gather and send to SNAP all paperwork, documentation, and medical reports related to the situation along with a nominal $10 processing fee.
- SNAP reviews the paperwork and arranges a follow up session with parents.
- SNAP and the parents develop an approach to work with the medical insurance company and other local and national resources to solve the problem.

The Medical Insurance Empowerment Checklist
One of the most important steps to the process is to provide SNAP with the paperwork, documentation and supporting reports necessary to help find the best ways to work with private medical insurance companies. Print out this handy Paperwork Checklist and be sure to make copies of the requested items and keep the originals in your files.
Generally, SNAP will ask for the following information to get started:
- Your name, address, phone number, e-mail, fax number
- Child(ren) name, age, sex, and kind of special need
- What kind of insurance coverage: individual or employer sponsored? HMO, PPO, or indemnity plan?
- Description of the situation or pre-approval request
- Summary of actions taken to address the issues
